Required Proof of Eligibility for Occupancy for All Family Members

1. Letter from Social Security Office: Current benefit statement for Social Security, SSI and or SSD (1-800-772-1213) or go in person to the Federal Building in Binghamton.
2. Pension award letter, if any, received monthly or annually.
3. Department of Social Services: Current benefit statement of all monies received from public assistance.
4. Pension award letter, if any, monthly or annually received.
5. If employed: Last 6 pay stubs.
6. Current verification of checking and savings accounts: Cash value and interest earned (current 6 months bank statements)
7. Cash value of stocks, bonds, certificates and dividends or interest earned.
8. Current verification of amount received for alimony or child support.
9. Current verification of amount paid for child care (only if you are employed, going to school or looking for employment).
10. Current prescriptions paid by you (1 years worth). Some drug stores offer individual prescription computer print-outs. (Elderly/Disabled/Handicapped only)
11. Social Security Card(s)
12. Birth Certificate(s)
